Health history updates can automatically be sent ahead of time when a patient is due for an update. Flex makes it simple for all important policies, health history forms, and new patient forms to be signed and completed.
Customizing forms is easy. You can build your own to match any of your current forms or start with a sample new patient and health history form. Offices can customize forms to fit their practice's specific needs, while meeting compliance standards with applicable laws, by selecting from suggested questions and arranging them in a desired order. Once saved, forms are ready for use, with options for digital signatures and sending forms by text or email. Practices can also request that a patient upload a photo of their drivers license, insurance card, or even a selfie which saves back into the patient’s record in Open Dental.
